LONG ISLAND TRANSITION COORDINATION SITE Transition 101 for Parents ************************************************************************************ Date: December 6, 2007 Location: ESB Sherwood Time: 7:00pm-9:00pm Audience: Parents of students with disabilities ************************************************************************************* This presentation will provide parents of students with disabilities fundamental information about Transition Planning and its implications for students' post-secondary success. This workshop will address SPP Indicators 8, 13, and 14. PLEASE NOTE: We regret that food and beverages cannot be provided. It is recommended that you bring beverages/snacks with you for half-for workshops. Full day workshops will have a 45-minute lunch break. For convenience sake and to ensure that workshops begin and end on time, it is suggested that participants bring a bag lunch and beverage

Make a right and follow Colin Drive to 15 Andrea Road and make left. Park in front BOCES lot on left and enter building through reception area. Sunrise Highway: to Nicolls Rd. north. Make left at first traffic light (Greenbelt Parkway) and follow Colin Drive to 15 Andrea Road and make left. Park in front BOCES lot on left and enter building through reception area.
WHEATLEY HEIGHTS (Western Suffolk BOCES) Lee Avenue, Wheatley Heights, NY Phone number: 242-1128 Long Island Expressway: Exit 50, Bagatelle Rd. south (left turn at light). Stay on Bagatelle for about 2 miles to the blinking red light. Make a left onto Lee Avenue. School is on corner of Lee and Bagatelle Southern State Parkway: Exit 36 north and travel approx. 3 miles. Follow signs which read “To Long Island Avenue – South 18th Street”. Make a left and cross railroad tracks until road turns into North 18th Street. Continue to the end, which is Lee Avenue. Make a left onto Lee. School will be on right side.

Ocean Ave. south to Woodside Ave. and make a left. Take to Route 112 and make a right. Approximately 1.5 miles make a left onto Barton Ave. (at traffic light). Take Barton Ave. to first traffic light and make a right onto Washington Ave. Make your first left onto Arthur and then your first right onto Phyllis Drive. Take almost to the end and make a left into the first parking lot entrance. Directions from Sunrise Highway (heading East): Sunrise Highway to Exit 53, Route 112. As you get off the Sunrise Hwy., get into the left turning lane and make a left at the first traffic light onto Rte. 112 heading north. At the second traffic light make a right onto Clark Street. Take Clark Street to the end and make a left onto Washington Ave. At first stop sign make a right onto Arthur Ave. and your first right onto Phyllis Drive. Take almost to the end and make a left into first parking lot entrance. Directions from Sunrise Highway (heading West): Take to exit 53, Route 112 and make the first right onto Phyllis Drive and second right into the parking lot.
